Slim, busty women know the problem. Either woman can close the front of the blouse comfortably, then she is too far overall and the shoulders slip on the upper arms.
If she sits properly at the back and shoulders, the buttons jump off the front. I have solved the problem for me so that under blouses I now always wear tank tops or those with spaghetti straps in a matching color and then either leave the blouse completely open or close only the bottom three buttons.
